Incoming Students

Welcome to LAS!  If your child is going to be a new student at Lincoln Akerman School, please print and complete this form and return it to the Health Office.  This is very important in order to take the best possible care of your child while they are here.  Thank you for taking the time to complete it.  There are hard copies available at the front office as well.

Anaphylactic Allergies

Please print, complete, and return all three forms to the LAS Health Office.  There are areas to be completed by the parent/guardian and by the healthcare provider.  There are hard copies available at the school.

Asthma

Please print, complete, and return the Asthma Action Plan to the LAS Health Office. There are areas to be completed by the parent/guardian and by the healthcare provider.  There are hard copies available at the school.

• Non-prescription medication form (for medications not stocked in the health office)
