School Year Requirements
A child new to Lincoln Akerman School is required to provide a copy of a physical examination within 12 months prior to the day they start school and a copy of their current immunizations. Required immunizations are found in the below link. If a medical or religous reason exists for a child not receiving recommended vaccinations, please complete the appropriate waiver. Please note, medical exemptions need to be signed by the pediatrician. Religious exemptions need to be notarized.
Children returning to LAS are also required to be up-to-date on their vaccinations. This typically happens for entrance into pre-k, kindergarten, and seventh grade. Parents will be notified if their child is in need of the required immunizations and will need to provide documentation of their child having received the vaccination on or before the start of the school year.
